Vial of L.I.F.E.

The Vial of L.I.F.E. is a FREE kit for you to store important information that emergency medical personnel (firefighters, paramedics and hospital staff) need in a time of crisis. By completing the Vial of L.I.F.E. form you can provide emergency personnel with vital lifesaving information even if you are unable to communicate. It is very important that you keep this information accurate, up to date and stored in the upper shelf of your refrigerator door.

What is in the Vial of L.I.F.E. kit?

It is important to have one vial per person in the household. You will receive:

• One vial with lid.
• One identification sticker that is placed on the outside of the vial.
• One magnet (one per household) that is plcaed on the upper right hand corner of your refrigerator.
• One information sheet. This is for:
  
• personal information,
  any illnesses that you have,
  medications and two persons to contact in an emergency.

This is folded and stored in the vial.

One release of information form. this form gives the Emporia Police Department permission to flag your address as aVial of L.I.F.E. participant. this way, emergencey personnel knows you have a Vial of L.I.F.E. before they get to your house. This can save precious time in an emergency. This form is left where you pick up your Vial of L.I.F.E. and forwarded to the Emporia Police Department.
